- Application For Work Permit Made Outside of Canada (IMM 1295)
- Labor Market Impact Assessment (LMIA) provided by the Employment and Social Development Canada (ESDC)
- Offer of Employment to a Foreign National Exempt from a Labour Market Impact Assessment (LMIA) or Offer of Employment ID number
- Document Checklist (IMM 5488)
- Use of Representative (IMM 5476) (see if this applies to you)
- Canada regional visa office required documents and documents required by your home country

- Current, valid passport
- Two photographs, with your name and date of birth written on the back
- A photocopy of the information page of your valid passport or travel document
- Proof of personal funds
- Family Information form (IMM 5645 or IMM 5707)
- Photocopy of marriage certificate
- Statutory Declaration of Common-law Union (IMM 5409) (see applicability)
